Automatic Withdrawal Authorization

We are pleased to offer you a new service---Automatic Withdrawal. Now you can have your periodic donation made automatically from your checking or savings account. And you won’t have to change your present bank relationship to take advantage of this service.

The Authorization Withdrawal Plan will help you in several ways:

v  It saves time, fewer checks to write

v  Helps meet your financial commitment in a convenient and timely manner

v  It saves postage

v  It is easy to sign up for and easy to cancel

Here’s how the Automatic Withdrawal Plan works:

v  You authorize regularly scheduled payments to be made from your checking or savings account. Then, just sit back and relax. Your payment will be made automatically on the specified date and proof of payment will appear on your statement.

v  The authority you give to charge your account will remain in effect until you notify us in writing to terminate the authorization.

The Automatic Withdrawal Plan is dependable, convenient and easy. It saves you time while showing your financial support for our agency and its mission. To take advantage of this service, complete the authorization form and return it to PossAbilities of Southern Minnesota, 1808 3rd Ave SE , Rochester , MN 55904 .
